Social Worker

Trondheim municipality is recruiting social workers for home-based services, a new project to develop integrated health services in people's homes. You will work in a multidisciplinary team, visiting patients and families to address housing, finance, work and social networks, while coordinating transitions between hospital and home care. A bachelor's degree in social work is required, with experience in complex patient cases and preferably background in rehabilitation, mental health, or housing-focused social work.
